Introduction

Section II is an alphabetical listing of client symptoms, client problems, medical diagnoses, psychosocial diagnoses, and clinical states. Each of these will have a list of possible nursing diagnoses. You may use this section to find suggestions for nursing diagnoses for your client.

• Assess the client using the format provided by the clinical setting.

• Locate the client’s symptoms, problems, clinical state, diagnoses, surgeries, and diagnostic testing in the alphabetical listing contained in this section.

• Note suggestions given for appropriate nursing diagnoses.

• Evaluate the suggested nursing diagnoses to determine if they are appropriate for the client and have information that was found in the assessment.

• Use Section III (which contains an alphabetized list of all NANDA-I approved nursing diagnoses) to validate this information and check the definition, related factors, and defining characteristics. Determine if the nursing diagnosis you have selected is appropriate for the client.
