• The folliculitides are divided into superficial and deep forms (Table 31.1).
• Follicular papules and pustules can be distinguished from nonfollicular lesions by the presence of a hair piercing the lesion in the former; if hair shafts are not apparent, then determining if the lesions correspond to the spatial pattern of hair follicles can aid in diagnosis.
• Common; characterized by follicular papules or pustules that are often on an erythematous base.
• Often pruritic, sometimes painful.
• Favors areas with terminal hairs, such as the scalp and beard; also common on the trunk, buttocks, and thighs > axillae and groin.
• The most common type is culture-negative/normal flora, followed by bacterial folliculitis caused by Staphylococcus aureus.
• Multiple, less common etiologies (Table 31.1), requiring a systematic approach for adequate diagnosis and treatment (Fig. 31.1; Table 31.2).

• DDx: acne vulgaris, pseudofolliculitis barbae, rosacea; Grover's disease in adults; pustular miliaria in children.
• The major forms of deep folliculitis are outlined in Tables 31.1 and 31.3.
• Lesions are characterized by firm, tender, erythematous papules or nodules that may measure up to 2 cm in diameter.
• Should avoid manipulating (i.e. squeezing) furuncles as may cause bacteremia or seeding of heart valves (endocarditis), brain (abscess), or bones (osteomyelitis).
• The ‘follicular occlusion tetrad’ is a term that was coined for the common association of acne conglobata, hidradenitis suppurativa, dissecting cellulitis of the scalp, pilonidal sinus; also called ‘acne inversa’.
• A common, chronic inflammatory disorder occurring most often in the beard area of men who shave.
• Favors males > females; African-Americans and others with darkly pigmented skin and tightly curled hair.
• The proposed etiology is tightly curled hairs that curve back into the skin when shaved (Fig. 31.8).
• Also, there is a genetic predisposition if carrying a mutation in the 1A α-helical subdomain of the hair follicle companion layer-specific keratin K6hf (K75).
• Lesions range from inflammatory papules and pustules to firm papules and keloidal scars (Fig. 31.9).

• Intervention during the acute phase of PFB is essential.
• Rx: while the easiest way to cure PFB is to stop shaving, this may not be practical; laser hair removal systems that are skin-color appropriate offer the best option for a more permanent solution.
• In addition, shaving techniques can be optimized (Table 31.4) and other treatments may provide some control (Table 31.5).

• Begins as a chronic folliculitis of the posterior neck and occipital scalp; with time, keloidal papules and plaques develop (Fig. 31.10).

• Seen almost exclusively in males of African descent.
• Lesions are usually pruritic and sometimes painful and disfiguring.
• Often there is patchy alopecia or complete hair loss; occasionally there are subcutaneous abscesses with malodorous draining sinuses.
• The sooner this condition is treated, the less likely it will become disfiguring.
• Rx: outlined in Table 31.6.
• Prevention is important, and patients should avoid mechanical irritation of the posterior hairline.
• A cutaneous disorder that targets the apocrine gland-bearing skin sites, particularly the axillae and groin.
• A chronic condition characterized by recurrent ‘boils’ and draining sinus tracts with subsequent scarring.
• Favors females > males and persons of African descent; onset at or soon after puberty.
• Initially inflammatory nodules and sterile abscesses arise in the axillae, groin, perianal, and/or inframammary areas; often very painful (Fig. 31.11A).

• With time sinus tracts (Fig. 31.11B) and hypertrophic scars develop (Fig. 31.11C); chronic, malodorous drainage also occurs.
• Complications may include anemia of chronic disease, secondary amyloidosis, lymphedema, fistulas, arthropathy, and the rare development of SCCs within the chronic scars.
• DDx: staphylococcal furunculosis, Crohn's disease, granuloma inguinale, mycetoma, and scrofuloderma (a form of tuberculous lymphadenitis with cutaneous extension).
• Rx: difficult to treat and no one perfect treatment exists; surgical excision the closest thing to a ‘cure’ (Table 31.7).
• Avoid incision and drainage, as may lead to further scarring and sinus tract formation
• In general, medical treatment is recommended in early stages; surgical treatment should be performed as early as possible once abscesses, fistulas, sinus tracts, and scars develop.
