29 ASTHMATIC ATTACK: WHAT TO LOOK FOR

Acute asthma: A disorder characterised by paroxysmal narrowing of the bronchi. The bronchoconstriction causes wheezing, shortness of breath and a tight feeling in the chest.

CXR FINDINGS IN ACUTE ASTHMA

In 95% of patients the CXR is normal. In a few patients the lungs demonstrate hyperinflation—i.e. generalised hyperlucency (increased blackening) with the domes of the diaphragm unusually low.

IS A CXR NECESSARY?1-4

image Very rarely. A CXR is not necessary in the vast majority of patients presenting with an acute episode.
image Caution…this is providing that the clinical diagnosis of asthma is unequivocal5,6. In small children an inhaled foreign body can cause wheezing.

WHEN IS A CXR USEFUL?

In four circumstances. When:

1. The diagnosis of asthma is equivocal.
image A CXR may reveal another cause for the dyspnoea (e.g. foreign body inhalation, see p. 201).
2. Pleuritic pain occurs.
image Pneumothorax and/or pneumomediastinum are recognised complications.
3. There is a pyrexia.
image Pneumonia can induce or increase symptoms.
4. Unexpected and unexplained clinical deterioration develops.
image Mucus plugging may cause a lobe to collapse.

ABPA = allergic bronchopulmonary aspergillosis

Aetiology/pathology

A rare cause of asthma. Spores of the fungus Aspergillus fumigatus are inhaled. A hypersensitivity reaction is induced. The bronchi contain hyphae and the walls of the large bronchi become damaged resulting in proximal bronchiectasis; the upper lobes are especially affected.

Clinical features

Asthma unresponsive to conventional treatment. Blood eosinophilia. Immediate skin reaction to Aspergillus.

The CXR

Recognition of the classical CXR appearances resulting from ABPA may be the first signal that the patient does not have simple asthma. If the proximal bronchi become dilated and filled with mucus this causes a CXR appearance which is referred to as the toothpaste shadow, or a finger-in-glove appearance (Figs 29.6 and 29.7). Pulmonary densities, representing areas of acute eosinophilic infiltration of the alveoli4 also occur. These shadows often come and go—they flit—sometimes spontaneously, sometimes in response to steroid treatment.
