• In a normal scalp, 90–95% of hairs are in anagen phase, 5–10% in telogen phase (Fig. 56.1).

• About 50–100 hairs normally shed daily.
• Alopecias can be categorized as diffuse vs. circumscribed, patterned vs. non-patterned, and non-scarring vs. scarring loss (Fig. 56.2).
• 80% of Caucasian men are affected by age 70 years.
• Women are less likely than men to have a family history of the disorder.
• Related to hormonal effects of dihydrotestosterone (DHT), converted from testosterone by 5-alpha reductase.
• Sensitivity of scalp hair to androgen hormones causes gradual miniaturization of hairs on the frontal/midline/vertex regions of men and the midline and crown of women (Fig. 56.3).

• In women, the frontal hairline is spared (except in the setting of virilization) and a Christmas tree pattern of widening of the hair part may be seen.
• May first become clinically apparent with superimposed telogen effluvium >> alopecia areata, especially in women.
• Should exclude hyperandrogenism (e.g. ovarian or adrenal source) in younger women or in women with signs of virilization (see Chapter 57).
• Rx: topical minoxidil 2% or 5%, finasteride (inhibits 5-alpha reductase) in men, hair transplantation (using occipital hair as the source).
• Sometimes a definable precipitating event ~3 months prior precedes diffuse shedding, leading to a reduced density of hair on the entire scalp and occasionally other areas of body hair (Table 56.1).

Some authors also propose vitamin B12 or iron deficiency as causes.

• Shed hairs are predominantly telogen hairs (Fig. 56.1).
• Generally complete hair regrowth occurs after months to years.
• Some women have chronic telogen effluvium without a definable cause.
• Rx: discontinue any potentially offending drugs, exclude thyroid abnormality and etiologies listed in Table 56.1.
• Autoimmune disease with increased T-cells present in the hair matrix.
• Associated with atopy and other autoimmune diseases (e.g. autoimmune thyroid disease, vitiligo, inflammatory bowel disease, autoimmune polyendocrinopathy syndrome type 1).
• Average lifetime risk for developing this disease is 1–2%.
• Circular to oval areas of alopecia that may progress to total scalp hair loss (alopecia totalis) or total body hair loss (alopecia universalis) (Fig. 56.4).

• May see exclamation point hairs at borders (Fig. 56.4).
• Positive pull test (easily extractable telogen hairs at periphery of oval areas of loss) correlates with active disease.
• Ophiasis pattern is a band-like pattern of loss along the temporal/occipital scalp (Fig. 56.4) that may be less responsive to therapy.
• Associated nail findings = nail pitting, trachyonychia >> brittle nails, onycholysis, koilonychia, onychomadesis.
• Rx (for regrowth of hair): high-potency topical or intralesional CS (e.g. 5 mg/cc); topical irritants (e.g. anthralin or tazarotene), topical immunotherapy (e.g. squaric acid dibutyl ester); in rapidly progressive disease, occasionally oral CS are given for a limited trial period (e.g. pulsed therapy over 2–3 months), topical minoxidil 5%.
• Self-induced twirling, pulling, and/or breaking of hair.
• May be related to a psychologic disorder or stress.
• Plucking of scalp hair results in patchy >> diffuse alopecia, sometimes in a wave-like pattern or centrifugally; hairs tend to be of different lengths.
• In difficult cases, can distinguish from alopecia areata by shaving a defined area of involvement and observing for regrowth.
• Rx: counseling and treat any underlying psychiatric illness (see Chapter 5).
• Most commonly secondary to a long surgical procedure.
• Generally alopecia seen on occiput as a solitary, oval patch.
• Chemotherapeutic agents (e.g. cyclophosphamide, doxorubicin, paclitaxel, etoposide) are common causes of anagen effluvium.
• Anagen effluvium can also be secondary to exposure to metals, e.g. arsenic, gold.
• Telogen effluvium can be drug-induced (see Table 56.1).
• Classically defined as loss of hair follicles with scarring (e.g. lupus erythematosus, lichen planopilaris) or an alopecia in which hair does not grow back (e.g. chronic, long-standing traction alopecia).
• Non-scarring alopecias that may eventually result in permanent hair loss include male and female pattern hair loss > alopecia areata.
• In secondary scarring alopecia, the hair is destroyed nonspecifically, i.e. secondary to burns, radiation dermatitis, cutaneous malignancy (see Chapter 100), sarcoidosis, morphea, necrobiosis lipoidica, infections (e.g. severe kerion), mucous membrane (cicatricial) pemphigoid.
• Most commonly observed in black women.
• May be related to the use of chemical hair relaxers or thermal relaxers (e.g. flat iron).
• Slowly progressive; centered on the crown/vertex and midline (Fig. 56.5).

• Symptoms may be mild or absent.
• Occasionally patients have secondary changes, especially crusting or pustules.
• Rx: mild disease – oral tetracycline plus potent topical CS; severe disease – oral rifampin plus oral clindamycin; occasionally intralesional CS if an inflammatory component is present.
• Women > men, more common in Caucasians.
• 50% may have associated lichen planus involving the skin, mucous membranes, or nails (see Chapters 9 and 58).
• Usually several foci of alopecia with loss of follicles and scarring centrally (may be clinically subtle); peripheral follicles having a central, keratotic plug and a rim of inflammation (pink to violet in color) (Fig. 56.6).

• Centered on the crown/vertex or midline, or predominantly affecting the frontal hairline and eyebrows (‘frontal fibrosing alopecia’) (Fig. 56.7), or rarely a diffuse pattern.

• Graham–Little syndrome = scalp alopecia, alopecia of axillary/pubic areas, and grouped spinous follicular papules on trunk/extremities.
• Rx: topical or intralesional CS, oral doxycycline, oral antimalarials (e.g. hydroxychloroquine), oral retinoids, oral mycophenolate mofetil (if severe).
• A type of chronic cutaneous lupus erythematosus.
• More common in adult women, especially black women.
• Minority of patients fulfill the criteria for systemic lupus erythematosus (see Chapter 33), but patients need to be evaluated at the time of diagnosis of DLE and followed longitudinally.
• Circular lesions of erythema, atrophy, dilated/plugged follicles, and scale as well as alopecia.
• As lesions age, central hypopigmentation and peripheral hyperpigmentation may appear, along with scarring (Fig. 56.8).

• May see similar-appearing lesions elsewhere (especially face and ears).
• Rx: discussed in Chapter 33.
• Favors young black men (see Chapter 31).
• Occipital scalp most common site, but sometimes involves the crown/vertex.
• Smooth, firm papules > pustules.
• Initial lesions resolve with alopecia and/or protuberant papules.
• Coalescence of lesions leads to alopecia.
• Rx: intralesional CS, oral antibiotics, excision (see Table 31.6).
• May be associated with ‘follicular occlusion tetrad,’ which includes this entity, hidradenitis suppurativa, acne conglobata, and pilonidal cyst/sinus.
• Affects young adult men, especially black men.
• Multiple, firm scalp nodules over crown/vertex/occiput that become boggy and fluctuant with purulent discharge; often interconnecting.
• Rx: intralesional CS, antibiotics based on cultures, incision and drainage, excision.
• Inflammatory scarring alopecia with perifollicular papules and pustules.
• Rx: dapsone, oral antibiotics (e.g. tetracycline or doxycycline for anti-inflammatory effects).
• Considered by most to be the end-stage, burned-out phase of different scarring alopecias (Fig. 56.9), especially lichen planopilaris.

• Occurs after years of hair styling that causes traction.
• Generally seen in black women on the bitemporal/frontal scalp line (Fig. 56.10).

• Four main categories: (1) fractures (trichorrhexis nodosa and invaginata, trichoschisis); (2) irregularities (monilethrix); (3) twisting (pili torti, woolly hair, trichonodosis); and (4) extraneous matter (Fig. 56.11).

• Can also divide into increased breakage (bubble hair, monilethrix, pili torti, trichorrhexis invaginata and nodosa, trichothiodystrophy) and no increased breakage (loose anagen, pili annulati, spun-glass hair, woolly hair).
• Trichorrhexis nodosa – common hair shaft abnormality that leads to increased breakage; hair shaft fractured with the appearance of two brushes pushing against each other; seen with hair straightening.
• Loose anagen hair syndrome – common cause of sparse, short hair especially in young children, with improvement over time; anagen hairs are poorly anchored and therefore easily shed or pulled, but breakage is not increased.
